Thalassaemia in Nigeria: Inheritance, Screening, and Living Well

By FXMed Team

A Blood Condition Many Nigerians Carry Unknowingly

Beta thalassaemia is an inherited blood disorder affecting haemoglobin production. Thalassaemia trait (carrying one abnormal gene) is relatively common in Nigeria — particularly in communities with historical malaria exposure, where the trait provided partial protection. Most carriers are unaware of their status — and the implications become critical when two carriers have children together.

Thalassaemia Trait vs. Thalassaemia Major

FXMed tip: If any of this sounds familiar, our Functional Health Analysis may be exactly what you need. Book a free assessment to find out where your health stands today.

  • Thalassaemia trait (minor) — one abnormal gene; typically causes mild anaemia or no symptoms; often discovered incidentally on a blood count (see our guide to understanding your blood tests) showing low MCV (small red cells)
  • Thalassaemia intermedia — two abnormal genes of moderate severity; causes moderate anaemia requiring monitoring
  • Thalassaemia major (Cooley's anaemia) — two severe gene mutations; causes life-threatening anaemia requiring regular blood transfusions from infancy; without treatment, fatal in early childhood

Why Carrier Screening Before Pregnancy (see our antenatal care guide) Matters

If both parents carry thalassaemia trait, each pregnancy has a 25% chance of thalassaemia major. Knowing this before pregnancy allows couples to understand their options and make informed reproductive decisions. FXMed strongly advocates for pre-pregnancy haemoglobin electrophoresis screening for both partners.

Optimising Health as a Thalassaemia Carrier

  • Iron supplementation is NOT routinely needed and can be harmful — check iron status before supplementing
  • Folate supplementation is beneficial — supports red cell production
  • Vitamin D optimisation for bone health (thalassaemia trait slightly affects bone density)
  • Avoid unnecessary iron (including iron-fortified foods in excess)
