Blood Tests: Why 'Normal' Doesn't Always Mean Optimal

It's one of the most frustrating experiences in modern healthcare: you feel genuinely unwell — exhausted, foggy, gaining weight, cold all the time, hair falling out — you have blood tests done, and the doctor tells you everything is normal. You're sent home with nothing.

This happens because of two specific problems with conventional lab testing: what gets tested and what "normal" actually means.

Problem 1: The Reference Range Trap

The "normal" range on a blood test is calculated from a statistical average of a large population — often an unhealthy population. If the average person's ferritin is between 15 and 200, that becomes the normal range, even though a ferritin of 18 can cause profound fatigue in many individuals.

Functional medicine uses optimal ranges — tighter windows associated with peak function, not just the absence of disease. The difference can be significant. A TSH of 4.5 is "normal" by most lab reference ranges, but many people feel significantly better with a TSH between 1.5 and 2.5. You'd never know from a standard result printout.

Problem 2: The Wrong Tests Are Being Ordered

A standard GP blood test in Nigeria typically includes a full blood count, perhaps a basic metabolic panel, and a few organ function markers. Useful — but it leaves enormous gaps. Here's what most standard tests miss:

  • Thyroid: Most tests only check TSH. A full thyroid picture requires Free T3, Free T4, Reverse T3, and thyroid antibodies. These tell you whether thyroid hormone is being produced, converted, and actually getting into cells.
  • Iron: Haemoglobin is checked. Ferritin (storage iron) rarely is — yet ferritin depletion causes fatigue, hair loss, and breathlessness well before anaemia develops.
  • Vitamin D: Essentially never tested in routine Nigerian screens, despite being deficient in the majority of urban adults.
  • Magnesium: Serum magnesium is an inaccurate measure (only 1% of body magnesium is in blood). Red blood cell magnesium is more meaningful. Deficiency drives anxiety, muscle cramps, poor sleep, and high blood pressure.
  • Insulin: Fasting blood glucose is checked — but insulin resistance can develop years before glucose rises. Fasting insulin tells you what's actually happening with your metabolic health.
  • Inflammatory markers: High-sensitivity CRP (hsCRP) is a far more useful cardiovascular risk marker than standard CRP, yet it's rarely tested.
