  • Food sticking in the throat or chest — suggest oesophageal obstruction or motility disorder
  • Difficulty initiating swallowing — may indicate neurological or pharyngeal (throat) cause
  • Pain on swallowing (odynophagia) — suggest inflammation, infection, or oesophageal spasm
  • Regurgitation of undigested food — classic for oesophageal motility disorders or strictures
  • Progressive difficulty starting with solids, later liquids — this pattern is particularly concerning and needs urgent investigation

Common Causes in Nigerian Adults

  • Achalasia — failure of the lower oesophageal sphincter to relax; common and treatable
  • Gastro-oesophageal reflux disease (GERD) — chronic acid damage causes stricture or Barrett's oesophagus
  • Oesophageal cancer — progressive dysphagia, especially in men over 50, is a red flag
  • Candida oesophagitis — common in immunosuppressed patients (HIV, diabetes, steroid users)
  • Iron deficiency — Plummer-Vinson syndrome; thin mucosal webs cause dysphagia, reversed by iron repletion
