When a Joint Becomes Infected
Septic arthritis — bacterial infection within the joint space — is an orthopedic emergency. Bacteria release enzymes that rapidly destroy cartilage: permanent joint damage can occur within 24–48 hours. In Nigeria, where STIs including gonorrhoea are common (gonococcal arthritis is the most common cause of septic arthritis in young adults) and where immune suppression from diabetes and HIV is prevalent, septic arthritis is a significant clinical concern.

- Single joint that is acutely red, swollen, warm, and exquisitely painful
- Severe pain with any movement of the joint
- Fever — though may be absent in immunocompromised patients
- Inability to bear weight on the affected joint
- Most commonly affects: knee, hip, shoulder, wrist, ankle
How to Distinguish from Other Joint Conditions
- Gout — typically affects the big toe; less likely to cause fever; responds to NSAIDs
- Reactive arthritis — follows an infection elsewhere (gut, genitourinary); less acutely severe than septic arthritis
- Rheumatoid flare — typically involves multiple joints symmetrically; less severe systemic upset
Why This Needs Urgent Medical Attention
Septic arthritis requires urgent joint aspiration (to confirm diagnosis and drain infected fluid) followed by intravenous antibiotics. This is not a condition to manage with herbal remedies or to "wait and see." Any acutely swollen, hot, painful joint with fever must be assessed medically the same day.
