The heart enlarges when it must work harder than normal over time, or when the heart muscle itself is diseased:
- Hypertension — the most common cause in Nigeria; the left ventricle thickens and eventually dilates under chronic pressure
- Cardiomyopathy — heart muscle disease; can be dilated (weakened, stretched heart) or hypertrophic

- Valvular disease — damaged heart valves cause the heart to work harder to compensate
- Anaemia — severe chronic anaemia forces the heart to pump more blood to compensate for low oxygen-carrying capacity
