- Pallor — pale gums, pale inner eyelids, pale skin; often the first sign parents notice
- Unexplained bruising — particularly in unusual locations (back, face, trunk) without trauma
- Petechiae — tiny, flat red or purple spots on the skin from microscopic bleeding; look like a rash that doesn't fade on pressure
- Persistent fever without clear infection source
- Bone pain — particularly leg pain at night; children may refuse to walk
- Lymph node swelling — neck, armpit, or groin; painless and persisting beyond 4 weeks

The Investigation That Makes the Diagnosis
A full blood count (see our guide to understanding your blood tests) (FBC) with differential is the first critical investigation. Abnormal white cell count, very low haemoglobin or platelets, or the presence of blast cells on blood film requires urgent haematology or paediatric oncology referral.
