• Stroke survivors — particularly those with dysphagia in the acute phase; swallowing function needs formal assessment before any oral feeding
• Patients with Parkinson's disease — progressive swallowing impairment is a major complication
• Elderly patients with dementia — cognitive decline impairs the coordinated swallowing reflex
• Patients with reduced consciousness — from any cause including sedating medications
• Newborns with tongue-tie or structural anomalies — aspiration during feeding
• Patients with severe gastro-oesophageal reflux — acid and stomach contents can be aspirated, particularly at night

Signs That Swallowing May Be Unsafe

• Coughing or choking during or after eating/drinking
• Wet, gurgly voice quality after swallowing
• Food or liquid coming back up through the nose
• Recurrent chest infections without obvious cause
