- People who consume polished white rice as a staple — milling removes 80% of thiamine; multiple daily portions with no compensatory sources creates risk over time
- Regular alcohol consumers — alcohol dramatically impairs thiamine absorption and increases requirements; most alcoholics become thiamine-deficient
- Pregnant (see our antenatal care guide) and breastfeeding women — requirements increase significantly; deficiency in nursing mothers causes infantile beriberi in their babies
- People with chronic digestive illness — Crohn's disease, prolonged diarrhoea, and bariatric surgery impair absorption
- HIV patients on antiretroviral therapy — some ARTs affect B vitamin metabolism
Symptoms of Deficiency
- Wet beriberi: Heart failure, swollen legs, rapid heartbeat — the cardiac form
- Dry beriberi: Peripheral nerve damage — tingling, weakness, pain in hands and feet
- Wernicke's encephalopathy: Confusion, eye movement abnormalities, unsteady gait — a neurological emergency requiring IV thiamine
