Short Stature: Raising a Child With the Most Common Form of Dwarfism-Achondroplasia
Understanding the Most Common Form of Dwarfism
Achondroplasia is the most common form of disproportionate short stature (dwarfism), affecting approximately 1 in 25,000 live births worldwide. It's caused by a specific mutation in the FGFR3 gene that slows bone growth in the long bones. The majority of cases (80%) are new (de novo) mutations — neither parent has achondroplasia. Intelligence is not affected.
Recognising Achondroplasia
- Short limbs (especially upper arms and thighs) with average-size trunk
- Enlarged head (macrocephaly) with prominent forehead
- Midface hypoplasia (flat nasal bridge)
- Average intelligence — achondroplasia does not affect cognitive development
Medical Issues to Monitor Actively
- Foramen magnum stenosis — the opening at the base of the skull can compress the brainstem; requires MRI screening in infancy. A neurosurgical emergency if compression is significant
- Sleep apnoea — extremely common due to midface anatomy; sleep study should be performed
- Middle ear dysfunction and hearing loss — recurrent otitis media is very common; hearing assessment needed
- Spinal stenosis — lumbar stenosis causes leg pain and weakness in adults
