Cachexia is a complex metabolic syndrome characterised by severe muscle wasting, loss of fat, weakness, fatigue, and loss of appetite, driven by underlying illness. Unlike starvation, where the body primarily burns fat, cachexia actively breaks down muscle mass through inflammatory pathways — and this cannot be reversed simply by eating more. It's seen in cancer, advanced heart failure, HIV/AIDS, chronic kidney disease, and severe infections.
What's Driving the Muscle Loss
- Pro-inflammatory cytokines — TNF-alpha, IL-6, and IL-1 from the underlying disease activate muscle protein breakdown
- Anorexia — cytokines suppress appetite centrally; the patient can't eat enough even if they want to
