• New or changing headache pattern — particularly headaches that are worst in the morning, wake from sleep (see our sleep health guide), or are progressive over weeks
• New onset seizures in an adult without prior epilepsy history — this is a brain tumour until proven otherwise
• Progressive neurological deficit — weakness, numbness, speech difficulty, or vision change that comes on gradually
• Personality or behaviour change — particularly frontal lobe tumours present with this rather than headache
• Cognitive decline — memory and concentration changes in previously sharp adults
• Nausea and vomiting associated with headache and not explained by gut cause

Investigation Pathway
