Inflammation That Shouldn't Be Ignored
Balanitis — inflammation of the glans (head) of the penis — is common, causing redness, swelling, soreness, itching, and sometimes a discharge. While it's often treated with antifungal or antibiotic cream and resolves, recurrent balanitis is a different matter — it almost always points to an underlying cause that needs addressing.
Common Causes
- Candida (yeast) infection — the most common cause. High blood sugar from diabetes provides an ideal environment for Candida. Many men first discover they have diabetes because of recurrent balanitis
- Bacterial infection — poor hygiene under the foreskin (particularly in uncircumcised men in humid Nigerian conditions) allows anaerobic bacteria to proliferate
- Skin conditions — psoriasis, lichen sclerosus, and contact dermatitis can all cause balanitis-like inflammation
- Sexually transmitted infections — gonorrhoea, chlamydia, and trichomonas can cause penile inflammation
When to Suspect Diabetes
Any man with recurrent balanitis — especially if he's over 40, overweight, or has a family history of diabetes — should have fasting glucose and HbA1c tested. In Nigeria, where millions of people are living with undiagnosed type 2 diabetes, balanitis is frequently the first presenting symptom of elevated blood sugar.
