Unusual Pneumonia: When a Chest Infection Won't Respond to Standard Antibiotics
When Standard Treatment Doesn't Work
Most pneumonia in Nigeria is treated empirically — typically with amoxicillin or co-amoxiclav. But a significant proportion of community-acquired pneumonia is caused by "atypical" organisms — Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila — that are intrinsically resistant to these antibiotics. These infections require macrolide (azithromycin) or tetracycline (doxycycline) antibiotics.

Atypical pneumonia often has a more gradual onset than typical pneumococcal pneumonia — sometimes called "walking pneumonia" because patients may remain ambulant despite being quite unwell:
- Persistent dry cough (not productive)
- Gradual onset over 1–3 weeks rather than sudden deterioration
- Relatively mild fever compared to the degree of chest involvement on X-ray
- Associated non-respiratory symptoms: headache, muscle aches, sore throat
- Poor response to initial penicillin-based antibiotics
When to Suspect It
Consider atypical pneumonia when: standard antibiotic treatment has been given for 48–72 hours without clinical improvement; pneumonia develops in a younger adult without comorbidities; chest X-ray shows bilateral patchy changes rather than lobar consolidation.
